Advance Care Planning Group Visits (ACP-GV) offer a place for you to share, learn and help your fellow Veterans understand the benefits of planning for future healthcare decisions if you become too ill to make or communicate choices for yourself.
ACP-GV can help you understand how to fill out an advance directive that matches your preferences so you are always in control of your healthcare.
